Self Pay & Insurance

Fee-for-Service:
- Most services require upfront payment (cash, card, HSA/FSA).
Insurance Claims:
- 4 Integrated Health directly bills United Healthcare, Aetna and BCBS.
- For other insurers, patients submit claims themselves using a superbill.
Blood Testing:
- LabCorp bills insurance directly.
- Patients are responsible for any remaining costs.
Saliva and Food Sensitivity Testing:
- Payment is always due upfront.
Check Insurance Coverage: Before making an appointment, confirm your insurance coverage with 4 Integrated Health and inquire about their current in-network status with your specific plan.
Understand Out-of-Pocket Costs: Be aware of potential out-of-pocket expenses, such as deductibles, co-pays, and any costs not covered by insurance.
Obtain Superbill: If you need to submit claims yourself, request a superbill from 4 Integrated Health to help with the process.
