At 4 Integrated Health, we operate on a fee-for-service basis, requiring payment at the time of service for office visits and saliva ​testing. We accept payments in cash, credit card, and HSA/FSA forms. If you choose to submit an insurance claim for your office visit, ​we can provide a receipt or superbill via our patient portal for potential reimbursement according to your policy.

We offer on-site blood testing services through LabCorp, which can directly bill your insurance for these tests. Any cost not covered ​by your insurance will be your responsibility to pay directly to LabCorp. Please review your insurance policy for details. If you ​prefer, ​blood tests can be sent to alternative labs, though you will remain responsible for the associated costs.
